About 10 percent of physicians use an EMR, and more than half are primary care physicians. A review of 363 malpractice claims from The Doctors Company revealed that system errors contributed to 30 percent of the claims. Thirty-two percent of these system errors were medication-related errors, 27 percent were communication errors, and 13 percent were medical record errors.

Medication errors involved monitoring (one-third were failures to properly monitor Coumadin), wrong dosage, inappropriate medication, failure to consider side effects, drug-to-drug interactions, and errors related to medication reconciliation.
